The sternoclavicular joint is the only true point of articulation between the upper extremities and axial skeleton. This articulation is limited to the sterno-clavicular and costo-clavicular surfaces. Sternoclavicular (SC) joint dislocations were first described in 1824, with multiple subsequent case reports. Anterior and posterior sternoclavicular dislocations are described based on the position of the clavicle in relationship to the sternum. Sternoclavicular joint dislocation is a common trauma condition to the shoulder. Injuries range from a separated shoulder resulting from a fall onto the shoulder to a high-speed car accident that fractures the shoulder blade (scapula) or collar bone (clavicle).

Posterior dislocation of the sternoclavicular joint is less common than the anterior type but is potentially much more serious. Damage to important structures located behind the sternoclavicular joint (arteries, veins, nerves, esophagus, trachea) can cause difficulty breathing and swallowing, poor circulation to the arm and hand, and nerve damage.

Once posterior dislocation of the sternoclavicular joint has been diagnosed, closed reduction should be attempted as soon as possible with a thoracic surgeon available. Most posterior sternoclavicular dislocations can be treated successfully with closed reduction and 6 to 8 weeks of immobilization.
